  • Your GP must assess you as having a chronic medical condition that has been, or is likely to be, present for at least 6 months.

  • Your GP must select item 10953 for Exercise Physiology services. Eligibility for up to 5 individual services per calendar year.

• Follow-up Allied Health Services for people of Aboriginal or Torres Strait Islander descent - GP referral Form

  • Your GP must have undertaken a health assessment and identified a need for follow-up allied health services.

  • Your GP must select item 81315 for Exercise Physiology services. Eligibility for up to 5 individual services (on top of 5x10953 item numbers for a total of 10 services per calendar year).
